La definición de hipertensión sistólica aislada (HTSA) aceptada por las guías internacionales es aquella que tiene como punto de corte la presión arterial sistólica (PAS) igual o mayor a 140 mmHg con valores de presión arterial diastólica (PAD) por debajo de los 90 mmHg. Este fenotipo es la forma más prevalente de hipertensión arterial (HT) en jóvenes. El pronóstico de la HTSA es diferente de acuerdo con diferentes mecanismos fisiopatológios. Se han descripto algunas relaciones de este fenotipo de presión arterial (PA) con las mediciones de presión arterial ambulatoria (MAPA), pero no se ha puesto relevancia en su asociación con los valores de la PA nocturna. En nuestro mayor conocimiento, no existen datos epidemiológicos en pacientes jóvenes ni estableciendo esta relación en Latinoamérica. Objetivo: Describir las características de la HTSA en jóvenes de población que consulta un hospital de tercer nivel en la provincia de Buenos Aires y analizar la relación con los distintos fenotipos de MAPA. Materiales y métodos: Se realizó un estudio de corte transversal con individuos adultos entre 16 y 40 años sin tratamiento antihipertensivo, derivados para la realización de un MAPA a un hospital de tercer nivel, desde el 1 de Julio de 2013 al 31 de agosto de 2022.Se realizó a cada individuo una entrevista inicial, toma de PA y evaluación MAPA. Resultados: Se evaluaron 395 individuos (69% mujeres, 29±6.5 años). Las prevalencias según fenotipos de consultorios fueron Normotensos (77%), HTSA (6%), HT (17%). De los HTSA se registró un 29.2% de hipertensión guardapolvo blanco (GB). El acuerdo Kappa fue de K= 31% para fenotipos de consultorio vs MAPA. El 37% de los HTSA tenía al menos un período elevado (diurno o nocturno), 12.5% solo el período nocturno. Discusión: Se encontraron prevalencias de HTSA menores a las descriptas en la literatura, la hipertensión sistodiastólica es el subtipo de hipertensión predominante aún en jóvenes en Argentina, a diferencia de otros reportes en otros países. La utilización del MAPA es importante para caracterizar correctamente al fenotipo y evaluar el pronóstico a largo plazo.
In EnglishThe definition of isolated systolic hypertension (ISH) accepted by clinical guidelines sets a systolic blood pressure (SBP) cutoff point at ≤140-90 mmHg, with diastolic blood pressure (DBP) values below 90 mmHg. This phenotype is the most prevalent type of arterial hypertension (AH) in young people. The prognosis of isolated systolic hypertension (ISH) varies depending on different pathophysiological mechanisms. Some associations of this phenotype with ambulatory blood pressure monotoring (ABPM) have been described, but its association with nocturnal BP values has not been emphasized. To our knowledge, there are no epidemiological data on young patients in Latin America establishing this relationship. Objective: to describe the characteristics of ISH in young patients consulting at third-level hospitals in the Buenos Aires province, and to analyze the relationship with different ABPM phenotypes. Materials and methods: adults between 16 and 40 years of age without antihypertensive treatment were evaluated in a cross-sectional study, derived for an ABMP to a third-level hospital, from 1 July 2013 to 31 August 2022. Each individual underwent an initial interview, BP measurements, and ABPM evaluation. Results: a total of 395 individuals were evaluated (69% women, 29±6.5years of age). The prevalence according to office phenotypes was: normotensives (77%), ISH (6%), and HT (17%). Among those with ISH, 29.2 % registered white-coat hypertension (WCT). The Kappa agreement was K=31% for office phenotypes versus ABMP. Of those with ISH, 37% registered at least one elevated period (either diurnal or nocturnal), and 12.5% had only the nocturnal period elevated. Discussion: lower prevalences of ISH were found compared to those described in the literature. Systolic-diastolic hypertension remains the predominant subtype of hypertension even in young people in Argentina, in contrast to reports from other countries. The use of ABMP is important for accurately describing the phenotype and assessing long-term prognosis.
