Maternidad y consumo problemático de sustancias: abordajes en un Hospital General de Agudos

Abstract

Inicié la residencia de Trabajo Social en un servicio social de un hospital general de agudos en CABA, zona sur. En dicho servicio les profesionales están divididos en tres equipos, de acuerdo, a las salas de internación en la que se encuentran interviniendo. Las mismas son la de clínica médica y traumatología, maternidad y pediatría. Durante el primer año de residencia vas rotando cada tres o cuatro meses por las distintas salas de internación con el objetivo de conocer e implementar la modalidad de intervención del Trabajo Social en dichas salas. Mi segunda rotación fue por la sala de maternidad. En ese momento, el equipo de Trabajo Social estaba interviniendo en la situación de una mujer que había ingresado al hospital por el nacimiento de su hije y se encontraba en situación de calle y consumo de sustancias psicoactivas. Como no conocía en profundidad sobre la temática ni tampoco las modalidades de abordaje de este tipo de problemáticas desde la sala de maternidad de un hospital, es que surgió el interés de escribir sobre dicha temática y, ante la complejidad de la misma, es que considero importante debatir y compartir reflexiones, desafíos a fin de repensar nuestras prácticas profesionales en pos de continuar garantizando a las personas con las cuales intervenimos el derecho a la salud de manera integral. Para la producción del trabajo se utilizó una metodología de tipo cualitativa. Se tomaron como insumos fuentes bibliográficas sobre la temática, observaciones realizadas durante mi inserción en la sala de maternidad y registros de historias sociales. Se realizaron entrevistas semi-estructuradas a una trabajadora social que forma parte del equipo de Trabajo Social, a una médica neonatóloga y a una residente de ginecología. En un primer momento, se desarrollará el marco conceptual con el objetivo de reflexionar sobre la concepción que prevalece sobre la maternidad y el consumo de sustancias psicoactivas. En un segundo momento voy a caracterizar las modalidades de abordaje del equipo de Trabajo Social y del equipo médico, que se llevan adelante en la sala de internación de maternidad, ante situaciones de mujeres que ingresan al hospital y se encuentran en situación de consumo de sustancias psicoactivas. Y analizaré si las concepciones sobre la maternidad y el consumo se encuentran presentes en las intervenciones de les profesionales de salud. Luego compartiré una experiencia de intervención del equipo de Trabajo Social a modo de continuar reflexionando sobre las prácticas profesionales. En un tercer momento voy a desarrollar los obstáculos que les profesionales de salud, que trabajan en la mencionada sala, identifican al momento de intervenir en situaciones relacionadas a consumo problemático. Y, por último, voy a compartir una conclusión a modo de reflexión.

I started the Social Work residency in a social service of a general hospital for acute care in CABA, south zone. In said service, professionals are divided into three teams, according to the hospitalization rooms in which they are intervening. They are the medical clinic and traumatology, maternity and pediatrics. During the first year of residency, you rotate every three or four months through the different hospitalization wards with the aim of learning about and implementing the modality of Social Work intervention in said wards. My second rotation was through the maternity ward. At that time, the Social Work team was intervening in the situation of a woman who had been admitted to the hospital for the birth of her daughter and was on the streets and consuming psychoactive substances. Since I did not know in depth about the subject nor the modalities of approaching this type of problem from the maternity ward of a hospital, the interest in writing about this subject arose and, given its complexity, I consider It is important to debate and share reflections, challenges in order to rethink our professional practices in order to continue guaranteeing the people with whom we intervene the right to health in an integral way. For the production of the work, a qualitative methodology was used. Bibliographic sources on the subject, observations made during my insertion in the maternity ward and records of social histories were taken as inputs. Semi-structured interviews were conducted with a social worker who is part of the Social Work team, a neonatologist and a gynecology resident. At first, the conceptual framework will be developed with the aim of reflecting on the prevailing conception of motherhood and the consumption of psychoactive substances. In a second moment I will characterize the modalities of approach of the Social Work team and the medical team, which are carried out in the maternity hospitalization ward, in situations of women who are admitted to the hospital and are in a situation of psychoactive substance use. And I will analyze if the conceptions about maternity and consumption are present in the interventions of health professionals. Then I will share an intervention experience of the Social Work team in order to continue reflecting on professional practices. In a third moment, I will develop the obstacles that health professionals, who work in the aforementioned room, identify when intervening in situations related to problematic consumption. And finally, I am going to share a conclusion by way of reflection.
