El pasaje de melancolía a manía: ¿otro costado de la defensa?

Abstract

Las categorías clínicas de la melancolía y la manía han sido y son en la actualidad objeto de múltiples abordajes. Consideramos importante recuperar el interés por investigar este capítulo esencial en la historia psicopatológica de las psicosis. La psiquiatría a lo largo de su historia se ha abocado al tratamiento y al estudio de las mismas. Por su parte el psicoanálisis se ha pronunciado describiendo sus variedades clínicas y proponiendo tratamientos posibles. Ambas disciplinas han coincidido en que estas categorías están relacionadas. Nos proponemos efectuar un recorrido a partir del nacimiento de la PMD, para luego situarnos en algunas conceptualizaciones que Freud hizo sobre la melancolía y manía. Con este propósito haremos hincapié en el concepto de defensa. Rescataremos este concepto para indagar el pasaje manía-melancolía. Podemos postular que en la teoría freudiana cada entidad clínica se distingue en cuanto a la defensa ante la pulsión en sus diversas conceptualizaciones. Concluiremos que en la melancolía la defensa se presenta bajo una modalidad, que caracterizamos como rudimentaria en relación a las otras entidades. Constatamos que el yo del melancólico se vería indefenso ante la ferocidad del superyó, pudiendo evitarlo momentáneamente a partir del vuelco hacia la manía.

The clinical categories of mania and melancholia have been and still are studied by multiple disciplines. We will retrieve this chapter in the history of the psychopathology. The psychiatry has dedicated to the study and treatment of these entities. On the other hand, the psychoanalysis has described these entities and has proposed possible treatments. Both disciplines agree that these categories are related. We will analyze the birth of the manic-depressive psychosis, to study then the different conceptualizations that Freud made about melancholy and mania. For this purpose we will emphasize the concept of defense to elucidate the switch from the melancholia to the mania. We part from the premise that in the freudian theory each clinical entity is distinguished, among other things, in terms of the way that the defense acts against the drive in its different conceptualizations. We will conclude that in the case of the melancholia the defense acts in a particular form, which we characterize as rudimentary in comparison to other entities. We claim that the ego of the melancholic subject is almost defenseless against the ferocity of the superego, and the only way that it can be avoided momentarily is by shifting to a manic state.
