Incidencia de los factores subjetivos en la adherencia al tratamiento antirretroviral en personas que conviven con V.I.H.

Abstract

Este trabajo se propone presentar el problema que dio lugar al proyecto de investigación “Incidencia de los factores subjetivos en la adherencia al tratamiento antirretroviral en personas que conviven con V.I.H”. Está comprobado que la eficacia de las terapias antirretrovirales está determinada, fundamentalmente, por lo que se ha dado en llamar “adherencia” de los pacientes a las mismas. En la práctica, se observa que a pesar de indicarse tratamiento con terapia antirretroviral (T.A.R.V.) el paciente no la inicia o la interrumpe. Los estudios existentes señalan como factores incidentes fundamentales los relacionados con las características del paciente y con la relación médico-paciente. Sin embargo, no se han encontrado estudios sistematizados desde una perspectiva psicoanalítica que aborde el problema de la adherencia como una respuesta subjetiva. El presente artículo señala la importancia de desarrollar estudios acerca de la incidencia de los factores subjetivos desde una perspectiva psicoanalítica, sosteniendo como hipótesis que estos factores en muchos casos explicarían los obstáculos que se presentan en el curso del tratamiento.

The purpose of this paper is to present the problem around which the research project “Impact of subjective factors in adherence to antiretroviral therapy in people living with HIV” was developed. It is proved that the efficacy of antiretroviral therapies is determined mainly by the adherence of patients to them. According with our experience, despite indication of treatment with antiretroviral therapy (ART) patients did not start or follow it. Recent studies refer to characteristics of the patient and the medical-patient relationship as the main causing factors. However, there are no systematic studies from a psychoanalytic perspective, to addresses the problem of adhesion as a subjective response. This work points at the importance of developing studies about the incidence of subjective factors considering the psychoanalytic theory. Therefore, our hypothesis affirms that these factors could explain some of the obstacles in the course of treatment of VIH.
