Numerous studies have shown that Malassez Epithelial Remains (MER) are considered reservoirs of stem cells with high differentiation capacity, which can be isolated allowing their culture in vitro for the formation of different tissues such as bone, cementum and structures such as Sharpey fibers. The objective of this work was to convey emerging findings related to apical pathogenesis and repair through the structure and function of MERs. Relevant investigations were selected, data were collected on SC cultures from the periodontal sac and inflammatory cyst from the Laboratory of Molecular Biology and Biotechnology. A 20-year-old male patient was selected, who presented a painless radiolucent image around the 4.8 retained tooth, with a diagnosis presumptive dentigerous cyst. The cyst enucleation and tooth extraction were performed according to the protocol. A biopsy of the lesion was taken, dividing it into two parts: one conserved in 10% formaldehyde, and the second derived to LBMB-FOLP, where sac explants were made to obtain MSCs using the cell culture and expansion technique. The similarity between the SC of the periodontal sac and cystic cells allows us to infer the presence of SC in MER.
